 This sim never ceases to surprise us with its bugs, just when I thought I had seen them all during these two years.

If you look at my line-up, three of my players went off the field due to substitution and injury, but only two subs went on the field.
The reason is that my RIM had the injury at the same time when the subs were activated (around 60th minute). It seems the sim subs the injured player first. In this case, the sub happened to be the same sub for my RF, so when it tries to sub the RF in the queue, it couldn't find the designated sub and stopped there.

I don't know how their codes are written, but I can see two possible solutions.
(1) When injury happens, check if normal subs are also supposed to be activated at the same time. If yes, do the normal subs first.
(2) When normal subs can't find the designated player, search the bench for the most suitable (whatever that means) player like it is currently done for injury. 		30/03/2013 23:00

 OK, I should amend what I said here. Apparently it is just an accounting bug. The RF that was supposed to be subbed out didn't actually leave the field.
On the other hand, this is still not as desirable as the solutions I proposed. Also, the unresolved sub was still counted, which prevented the third sub from happening.

 Its happened before with me as well.

Played a friendly with Godson and I had subbed 3 players at 60 mins. They left the field but the new ones never came on. In the Line Up it showed arrows next to the three players that left but no Substitutions. 		31/03/2013 05:57

 this happened to me last season in a game with J.A. Camacho, he subbed off 3, and they never left. Unfortunately, it resulted in him playing low average players against me and almost pushed him into relegation. 31/03/2013 05:58

 last season i noticed a simular situation

but on checking at the end of game i had an injured player go off and another come on to replace him

however, as it was so late in game the replacement didnt get a rating or even show up in the line up as coming on

dont suppose this is your bug but i am not going to watch your sim - i dont even watch mine lol (unless i lose or there is an issue)
